To qualify for insurance reimbursement, you'll need a prescription from your doctor. Here's how:
  1. Schedule a visit with your licensed healthcare provider (dermatologist, oncologist, or primary care physician).
  2. Ask for a written prescription using one of the following terms:
    • Cranial Prosthesis
    • Hair Prosthesis
    • Cranial Hair Prosthesis
  3. The prescription should also include a diagnosis code (such as alopecia areata, chemotherapy-induced alopecia, etc.).
  4. Avoid using the word "wig" in the prescription—insurance companies may consider it cosmetic and deny coverage.
To submit your insurance claim, gather the following:
  1. A prescription from your doctor (with medical necessity and diagnosis code)
  2. A paid-in-full invoice from Divine Touch Hair Prosthesis
  3. A completed CMS-1500 Claim Form (we can help you with this)
  4. Insurance-specific codes:
    • Synthetic Wig: A9282 or 99199
    • Human Hair Wig: L8499
